Job summary
The role will coordinate and provide essential additional support to some of our most frail patients, supporting the development of expert frailty services in the community. The role demands thorough clinical assessment skills and a person-centred approach to formulate personalised comprehensive care plans which aim to keep patients at home for longer, reduce acute admissions and prevent hospital re-admission.
The postholder will be an autonomous clinician who will use their experience and expertise of working with frail elderly patients in a community setting, to coordinate the provision of care for our frail patients.
A key member of the Frailty team, you will have proven clinical and communication skills, be confident in working alone, and experienced in providing a holistic and comprehensive clinical assessment of your patients. Your role will be vital in communicating the needs and wishes of the patient to those involved in their care.
With this being a new role, you will work closely with a senior Band 8a nurse, supporting the development of the Frailty Nurse led services in North Powys.
